My Planned Care

If you are currently waiting for a hospital appointment, operation or treatment, we wanted to let you know that the NHS has recently launched the My Planned Care Patient Digital Platform www.myplannedcare.nhs.uk which gives you direct access to the latest average wait time information for your trust, as well as helpful advice and support whilst you wait.
We encourage to look at this site where you may find the information you need before contacting your GP or hospital for an update about any elective procedures you are waiting for.
Updated weekly, the site is easy-to-use and is ‘open-access’ so your carer, friends, relatives and your NHS team can also access this information.

                                                              

Telephone Advice

telephoneIf you feel it would help to speak to a doctor or one of the nurses over the telephone regarding a problem please speak to a Care Navigator. It would help the doctor or nurse to respond appropriately if you could outline the nature of your problem to the Care Navigator. Please ensure you give an up to date contact phone number to the Care Navigator. The doctor or nurse will call you back as soon as necessary or in the next few days. Alternatively they will suggest that an appointment is made.
