Covid Vaccine Passport/Certificate PLEASE READ

We are unable to issue this certificate at the practice.

Proof of your vaccination status will be available on the NHSapp, (Which is also valuable for accessing your health records and ordering repeat prescriptions)

Alternatively you can call the NHS helpline on 119 (from 17 May) and ask for a letter to be posted to you. This must be at least 5 days after you’ve completed your course of the vaccine, the letter may to take up to 5 days to reach you.

Telephone Advice

telephoneIf you feel it would help to speak to a doctor or one of the nurses over the telephone regarding a problem please speak to a receptionist. It would help the doctor or nurse to respond appropriately if you could outline the nature of your problem to the receptionist. Please ensure you give an up to date contact phone number to the receptionist. The doctor or nurse will call you back as soon as necessary or in the next few days. Alternatively they will suggest that an appointment is made.
