My Planned Care

If you are currently waiting for a hospital appointment, operation or treatment, we wanted to let you know that the NHS has recently launched the My Planned Care Patient Digital Platform which gives you direct access to the latest average wait time information for your trust, as well as helpful advice and support whilst you wait.
We encourage to look at this site where you may find the information you need before contacting your GP or hospital for an update about any elective procedures you are waiting for.
Updated weekly, the site is easy-to-use and is ‘open-access’ so your carer, friends, relatives and your NHS team can also access this information.

Wearing a face covering

With the recent change in government restriction guidelines to COVID-19, we ask that all patients attending the surgery continue to wear a face covering. This is for your protection as well as our staff.

Covid Passports

If you are aged 16 or over your Covid passport can be found on either the MyGP app, Patient Access app or the NHS app or if you do not have a mobile phone you can ring 119 and they will organise a letter to be sent to you.  If you are travelling please ensure you leave enough time to get this as it is sent out via the postal service.

We CANNOT print off Covid Passports - they are either electronic on your app or on paper via NHS 119

We do NOT provide "proxy" access for children aged between 11 and 15 and NHS 119 will not provide a letter either.



Making an appointment

If you feel you need to speak with a clinician for an urgent matter please ring the surgery on 01606 544644 and the Care Navigator will make you a telephone appointment for the same day. The clinician will ring you and if they feel you need to be seen face to face they will make this appointment for you.

If you want to speak with a clinician but it is not urgent for example a medication review, you can either ring or go on your app to make a "Phone Only" appointment with a clinician of your choice.

All requests for extension to sick notes must be requested via eConsult.

When you come to the surgery

Wherever possible you must come alone

You must wear a face covering (this can be an actual face mask or a scarf – it must cover your mouth and nose)

Please do NOT come to the surgery if you have any of the following (unless asked to do so by a clinician):

High temperature

New cough

Loss of taste/smell

Tested positive for Covid-19 in the last 14 days

Been in contact with somebody in the last 14 days who has had any of the above symptoms

When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
