Urgent and Routine appointments

For ALL urgent and routine appointments that you think you might need GP input you need to submit an eConsult form to Swanlow.

By having a total triage system, will allow more patients to gain contact with a clinician and see the right person for your condition first time.  The clinician looking at your form will personally it. They will decide the most appropriate pathway to help with your request.

You will receive contact the same day you submit your online form. This will be either by the triaging clinician, or by our admin team who will contact you.

We will always try to provide continuity for ongoing issues. This means that where possible we will book your appointment with a clinician who has already treated you.

Online eConsult Form

To submit an online form you can visit www.swanlowsurgery.nhs.uk

On our homepage there will be a green banner which states ‘Contact us online’. For GP appointments (both urgent and routine) please select ‘I need medical help’. You will then answer a few questions which will be looked at by a clinician to assess how urgent your request is.

If you are unable to complete a form for personal or technical reasons, our Reception Team WILL help. We will continue to provide support and guidance when needed with this system. All patients are still be able to access appointments when needed. You ARE still able to call Reception to ask for help.

  • To cancel your appointment:
  • Use your NHS account via the NHS App or the NHS App website.
  • Call us on 01606 544644, pressing option 1.
  • Reply CANCEL to your appointment reminder text message.

If you need help after surgery hours

If you need urgent medical help when we are closed you can use NHS 111 online or call 111 for free.

NHS111 online is for people aged 5 and over. For children under 5 please call 111 direct.

For life threatening emergencies, including mental health crisis please dial 999 immediately.

If you need help with an appointment

If you need further support or any reasonable adjustments, please ensure our team are aware to help accommodate you.  We can add this to your medical records so we are aware for future contacts you make with us.

If you would like to see a certain clinician our Reception Team will always accommodate where possible.

Your can either be a telephone or face-to-face appointment.

If you need an interpreter, please let the team know.

Home visits

Home visits are only for patients too ill to attend the surgery.

Winsford GP surgeries use a Rapid Response team for all urgent home visits. This service is ran using Advanced Clinical Practitioners who can diagnose, prescribe, and refer.

If you feel you need a home visit, please contact the surgery as soon as possible.

Medical Reports

We do not offer GP appointment for completion of medical forms or reports.

Any requests for GP letters, forms or medicals need to be requested via our Secretary Team. Please contact the team on 01606 544644 and press option 3. The team will then assist you further with your request.

PIP/ESA

Swanlow Medical Centre no longer provide letters to support with PIP/ESA claims. We can provide you with a medical summary to send with your claim. DWP will request further evidence if needed.
